Internal Server Error
occurs when i try to open
RequestAccount.php
WithGrid.php
controller.php

where is the problem ?

all the files in same folder.

I have not block error message:
RequestAccount.php shows
Fatal error: Class 'app\specs\SimpleForm' not found in /var/www/row-simple-form/RequestAccount.php on line 10

I think you have to make directory structure by RequestAccount.php lines 3-8.
But Options, models, Output and Email lack in this package :(

SimpleForm.php shows empty page.

This package is useless.
